Univera Senior Choice members are eligible for $650 annually for memberships
at any health club, gym or fitness facility.
There are two ways to receive reimbursement. Receive an Active Anytime Debit Card or
select the Univera Healthcare Medicare Advantage Plan, pay for services up front
and then file a paper claim for reimbursement.

FitBlue is an exercise and wellness program available to select BlueCross BlueShield of WNY Medicare Advantage HMO and PPO plans.
Eligible members must first register
for the benefit via email or phone,
and then receive a swipe card for use at the gym, which requires a ZERO co-pay!
The benefit offers a month-to-month fitness membership, acupuncture, chiropractic, and massage therapy services.

The Aqua benefit provides members
an individual or family annual allowance,
which can used toward a variety of services.
Individuals receive $250 and
families receive $500 per calendar year.

Many BlueCross BlueShield of WNY
cardholders are entitled to a 90-day fitness
membership with just a $25 co-pay.
Members who qualify can extend their gym membership following the 90-day term.
